Emergency information- In case of emergency each student is required to have on file at the school office the following information:

Parent(s) or guardian(s) names(s).
Complete and up-to-date address.
Home phone and parent(s) work phone.
Emergency phone number of friend or relative.
Physician’s name and phone.
Medical alert information.

We are finding that we do not have current phone numbers and no emergency phone numbers. If your number changes please contact the school with the updated information.

Medication- There may be a need for your child to take medication. If possible we do ask for the medication to be given at home.

There is a medication administration form that would need to be dated and signed.

These forms are available at the school or on the school website. The medication needs to be brought in by an adult and in the original box/packaging with the students’ name written on it. There are some medications that only need to be taken every 12 hours, this is a good choice as it doesn’t interrupt the school day.

Dental Letter to Parents

Dear Parent/Guardian:

The Marion County Public Health I-SmileTM @ School program is offering no-cost dental services for children at your school.

The following services may be provided by dental hygienists:
 Dental Screening – a simple look in the mouth to check for cavities.
 Fluoride Varnish – a sticky liquid that coats all teeth to help make them stronger and prevent tooth decay.
 Dental Sealants – a tooth-colored coating that is painted on the back teeth to protect them from food, germs, and acid that cause tooth decay.
 Oral Health Education – lessons to help students learn about healthy teeth and mouths.

After the services are provided to your child, a letter will be sent home to you with the results. Please note that dental screenings do not replace regular dental check-ups. Your child should visit the dentist for a complete exam.

Tdap Vaccination Requirements

Students entering 7th grade will be required to receive a one-time dose of tetanus, diphtheria, and acellular pertussis (Tdap) vaccine. Tdap can be given regardless of the interval since the last tetanus and diphtheria toxoid-containing vaccine. Approximately 75% of all 13-17 year olds in the United States have already received this vaccine. However, prior to this change, Iowa was one of only 10 states that did not have a Tdap vaccine requirement for secondary school.

Please provide documentation that your incoming 7th grader has received this vaccine anytime between now and the start of school in August. You may turn this documentation into the Lakeview office or have it faxed to us at 856-0641. If your student is in need of receiving the vaccine, it is available from your local primary care provider or Appanoose County Public Health.
